Check-out Cell Phones
To checkout a cell phone, the Cellular Phone Request Form should be completed and returned to the Services Manager with at least 24 hours advance notice. The form must include requested plan, PSU account number, authorized signature, destination and anticipated usage.
Rates are determined from the time the cell phone is picked up until it is returned to OIS. There is a minimum six-hour check-out period. The rates for the various plans are detailed on the Cellular Phone Request Form. Any roaming or long distance charges made during the check-out period will be passed on to the user.
Billing transactions will occur on the monthly telephone billing processed by OIS.
