State Employee Health Plan Online Open Enrollment Information for Plan Year 2016

The State Employee Health Plan posts all open enrollment materials on their website at www.kdheks.gov/hcf/sehp.htm to make the information accessible 24/7.

For Plan Year 2016, those members that would like to receive printed materials can go to www.surveymonkey.com/s/OEmaterials and complete the required fields by 06/26/2015.

New Electronic PSU Directory Goes Live

You may have noticed that the electronic PSU directory had been very slow to open for quite some time.  Sometime mid-January, it unilaterally “checked out” and no longer functioned.  OIS was aware of the decline in the directory response, and a few months ago, staff started working on new functionality.  We now have two new electronic processes for the electronic PSU directory.  You will find new processes on the PSU web page under the “University Directory” and “Find People” links found at the bottom of the home page.

HealthQuest Rewards Incentive Program - Free Biometric Screenings

HealthQuest Rewards Incentive Program began a new year November 16, 2014 through November 15, 2015. Employees can earn credits for a discount on their 2016 health insurance premiums! Click here for step-by-step instructions on how to make an appointment.

Employees only need to sign up for 1 screening during the period – November 16, 2014 through November 15, 2015.  Log in to your account on the wellness portal at www.KansasHealthQuest.com and click on "Biometric Screenings" from the left menu and then select "Onsite Screenings." Here are the dates for the Biometric Screenings on the PSU Campus:

  • May 28, 2015 - 7am-12noon - PSU, Overman Student Center (302 East Cleveland, Pittsburg, KS 66762) in the Governors Room

  • August 27, 2015 - 7am-12noon - PSU, Overman Student Center (302 East Cleveland, Pittsburg, KS 66762) in the Governors Room

Please note that the next HealthQuest program year began on November 16, 2014. Employees can report their appointments (doctor, dental and vision) taking place during the extension period (August 1 - November 15) for credits during the next program year. They just need to wait until November 16  when the new program year begins to report these on the wellness portal  at www.KansasHealthQuest.com.


Flu Shot Clinic

Employees and covered spouses with the State of Kansas Group Health Insurance may sign up to receive a free flu shot. Here are the flu shot clinic dates:

  • September 15, 2015 from 9am - 3:00pm in the Overman Student Center, Governor's Room

  • October 7, 2015 from 9am - 3:00pm in the Overman Student Center, Governor's Room

How to schedule an appointment:

  1. Click here.
  2. Company User ID: 5394
                   Password: 5394
  3. Create an account if you haven't already used the OccuVAX online scheduler.
    Employees that previously used the OccuVAX online scheduler will already have an account and should skip this step. A forgot password is available for those that need it.
  1. Select your clinic.
  2. Select an appointment time.
  3. Enter requested information.
  4. Click the Schedule Appointment button.
  5. You DO NOT need to pay for your flu shot!
  6. Print out the 2015 Consent Form for Insurance Claims. This is the form that you will take with you to your appointment and give to the nurse.


W-2 Information

W-2 forms for calendar year 2014 will be available electronically through the State of Kansas Employee Self Service Portal.  This option allows you to securely receive your W-2 electronically.   

If you consented to receive your W-2 electronically last year, that consent is still in effect.  If you would like to withdraw that consent and have a W-2 mailed to you, you must log onto the Employee Self Service Portal to withdraw consent.   Employees that would like to add electronic consent must log onto the Employee Self Service Portal and consent to receive this information electronically.  

Printed 2014 W-2 forms, generated for any employee who did not consent to receive W-2 information electronically through employee self-service, were released to the US Postal Service for delivery on Wednesday, January 28, 2015. 

Duplicate W-2s for 2014 will be available starting Wednesday, February 11, 2015.  Employees wishing to obtain immediate access to W-2 information, rather than requesting and waiting for a duplicate W-2 in the mail, can consent and receive their W-2 information electronically via Employee Self Service.  Employees wishing to receive a printed duplicate W-2 can submit a W-2 Reissue Request via Employee Self Service.
