All Health Savings Account and Health Reimbursement Account members should have received an Optum payment card. This card should be used instead of the US Bank Card.

New Long Term Care (LTC) Program On-Site Meetings and Webinars

The State of Kansas is pleased to announce two special benefits being made available to you as a State of Kansas Employee. The two programs being offered as voluntary benefit options designed to address your long term care risks: Long Term Care Insurance and Lifetime Benefit Term Insurance. These benefits are brought to you by ACSIA Partners, one of the nation’s most experienced professionals in long term care planning. Long Term Care and Lifetime Benefit Term Insurance are now available to you as voluntary benefits, because we understand the impact of not planning for extended care events, which are NOT covered by Medicare or traditional health and disability insurances. Take advantage of your one-time opportunity to apply for coverage with simplified underwriting. 

You are encouraged to attend an on-site meeting or educational webinar, which will explain all the benefits and options now available to you. Following the webinar, there will be an opportunity to indicate interest in a personal consultation with a Long Term Care Insurance Specialist, who can meet with you, your spouse, and extended family members.

Other important information to share about this new program:

  • A dedicated website is available for you at  Once on the landing page, your employees should click the “State of Kansas Employees” gold button for information specific to SOK employees.
  • Any previous LTC policies that employees may have purchased in the past are still valid with the prior carrier (for example, Genworth) provided the employee continues to pay their premiums directly to the carrier as they have been doing.  This new LTC program is not replacing those policies.  Employees that may have current LTC policies are welcome to find out information about this new offering distributed through ACSIA Partners.
  • The Initial Enrollment period for this new offering with Simplified Underwriting is March 1 through April 30, 2016.

You cannot join the webinar until you register. Once you arrive at the registration page, please click the "Register" button in the middle of the page. After you register, you'll receive an email that includes all the webinar details including a link to the webinar, a phone number to dial into, and the webinar password. Save the email so you'll have the information you need to sign in on the day you selected. You must dial-in to the provided conference number in order to hear the audio portion of the webinar. You can also find the dial in by clicking on the "I will call in" button once you join the screen share.

2015 W-2 Forms

W-2 forms for calendar year 2015 are available electronically through the State of Kansas Employee Self Service Portal.  This option allows you to securely receive your W-2 electronically.  

If you consented to receive your W-2 electronically last year, that consent is still in effect.  If you would like to withdraw that consent and have a W-2 mailed to you, you must log onto the Employee Self Service Portal to withdraw consent.   Employees that would like to add electronic consent must log onto the Employee Self Service Portal and consent to receive this information electronically.

Please note, printed W-2 forms, for employees who do not consent to receive their W-2 information electronically in ESS, will be mailed the last week of January in accordance with IRS regulations.

2015 IRS 1095-C Forms

The State Employee Health Plan (SEHP) requested that we provide information about the 1095-C Form.

The State of Kansas is required to provide the 1095-C Form to all employees who were eligible for coverage in the State Employee Health Plan (SEHP) for one or more months of the calendar year.  This Form is provided to all eligible employees, even those who declined or waived SEHP coverage in 2015.  The 1095-C Form is similar to W-2 or 1099 Forms that you will need to retain for your tax records and the preparation of your tax return.  More information on the 1095-C Form may be found on IRS websites here and here.

Your 1095-C Form is available in the state of Kansas Membership Administration Portal (MAP).  This portal is used by employees to make SEHP open enrollment elections and to request mid-year changes.  You will use your PSU active directory ID and password to sign in to MAP.  Once logged in, the 1095-C Form is located under the “Forms” tab, under “Tax Forms.”  The SEHP stated they also will mail this Form to the primary member covered under the SEHP at that member’s preferred mailing address which is located in the “Member & Family” tab in MAP.

Employees only need to sign up for 1 screening during the period – November 16, 2015 through November 15, 2016.  Log in to your account on the wellness portal at and click on "Biometric Screenings" from the left menu and then select "Onsite Screenings." If you have problems with their online scheduling system, please call their Help Desk to schedule an appointment at 1-888-275-1205, Option 5.
