W-2 Reprints

The State of Kansas mailed 2011 W-2's to employee's home address on January 5, 2012.  If you did not receive your 2011 W-2, you can request a duplicate from the State of Kansas.  Click on the following link for detailed instructions - http://www.pittstate.edu/office/hr/payroll-information/w-2-.dot

 

Free Health Screening

HealthQuest and the State Employee Health Plan (SEHP) are offering free health screenings at Pittsburg State University in the Balkans Room, Overman Student Center on Monday, February 27 through Wednesday, February 29, 2012. An appointment is required, and no walk-ins will be accepted. Appointments can be scheduled between 7 a.m. and 12 noon.

Results from the screening can be used to complete the health assessment. Employees must complete the health assessment and earn a total of 20 credits by July 31, 2012 in order to receive a $480 premium discount in 2013. Benefits eligible State employees who are enrolled in the state employee health plan or who have waived coverage in the plan and retirees, spouses and dependents 18 years or older who are enrolled in the State employee health plan are eligible to receive a free health screening.

Log in to your account (or register for one if you do not have an account) on the wellness portal at www.KansasHealthQuest.com and click on "Health Screenings" from the left menu and then select "Onsite Screenings."
