HOW & WHERE TO TURN IN FORMS:

Groups must bring one original and one copy of the entry form to the Campus Activities Center.  Both the original and the copy must be stamped with the Campus Activities Center stamp, dated, and initialed by a Campus Activities Center employee or Homecoming Committee member.  The original will be left at the Campus Activities Center and the copy will be retained by the group in the event there are any questions or problems. 

The Campus Activities Center will not make or provide any copies of the original form nor will any employee stamp, date, or initial an original without a copy.

The Campus Activities Center is located on the second floor of Hartman Hall.

OTHER INFORMATION:

All entry forms are DUE to the Campus Activities Center on Tuesday, September 9, 2014 by 4:00 pm.  Forms may be turned in starting at 8:30 am on Monday, August 25, 2014.

FORMS SHOULD BE TYPED.

In addition to the form, you must have the following information:

Kings & Queens - A parade form for either a car or a float.  ALL ORGANIZATIONS MUST PROVIDE PARADE TRANSPORTATION FOR THEIR CANDIDATES!

    Please note that destruction of a puzzle piece will require a replacement be supplied by the responsible organization.

Yell Like Hell - Fully edited CD with MP3 file - MUST BE ITUNES COMPATIBLE
