Request Paper Application to Apply for Provisional License

Provisional Teaching Endorsement License excluding special education.

If you a) hold a currently valid Kansas teaching license, b) can verify employment in an assignment to teach in the subject area/teaching field in which you are seeking a provisional license, and c) have completed 50% of any of PSU's subject area/teaching field programs excluding special education, you may apply for a two-year provisional license.

Provisional Teaching Endorsement License in special education.

If you a) hold a currently valid Kansas teaching license, b) can verify employment in an assignment to teach special education, and 3) have completed PSU's special education block that includes course work in characteristics and methodology and a practicum, you may apply for a two-year provisional license.

Provisional School Specialist (Library Media, Reading Specialist, or School Counselor) License

If you a) hold a currently valid Kansas professional teaching license, b) can verify employment in a school specialist assignment for Library Media, Reading Specialist, or School Counselor, and c) have completed 50% of PSU's program, you may apply for a two-year provisional license.

The FORM 4 Application for Kansas PROVISIONAL TEACHING LICENSE OR PROVISIONAL SCHOOL SPECIALIST LICENSE is a "paper" application only and unavailable on the KSDE web site.  You therefore must contact PSU Teacher Licensing, to request the FORM 4 application when you are prepared to apply for a Provisional Teaching Endorsement License or Provisional School Specialist Endorsement License and have an offer of employment in the assignment in which you are seeking a provisional license.

When your request for a FORM 4 application is received, you will be provided detailed instructions for completing the provisional application process and the application will be attached to an e-mail response.

Provisional Teaching Endorsement License or Provisional School Specialist Endorsement License is valid for two years unless the teaching license to which the provisional license is attached expires earlier and in which case the time remaining will be added when the teaching license is renewed.  If needed, a second two-year provisional license may be issued by 1) submitting an application for renewal, 2) providing verification of continued employment in an assignment covered by the provisional license, and 3) providing an official transcript verifying a minimum 50% program deficiencies were completed within the two-year validity of the first provisional license.  If you are eligible for provisional license renewal, please again request by e-mail or telephone the FORM 4 application.  Please note a second provisional license will be issued only if course work remains to be completed during the validity of the second license.  A second provisional license will not be issued if all course work has been completed and only the PRAXIS II content/subject assessment(s) remain to be taken or passed.
