Faculty Workshop Registration Form for 2011 Fall
1. Check the workshops below that you wish to attend and complete your contact information.
2. Option One: When through, click 'Submit Registration' at the bottom of the page. Your email client will open a new email containing your registration information. If your information is correct, send the email.
Option Two: If your email is empty after clicking 'Submit Registration', copy and paste the form on the screen into that email and send it.
3. I will post your name on the schedule when I receive your email.
Please mark your calendar for the workshop(s) as a reminder email will not be sent out.
4. If you cannot make it to a workshop that you have signed up for, please email me at pintar@pittstate.edu so I can open the seat up to another faculty member. Thank you.
